An inquiry revealed that a former mental health patient under the care of Nottinghamshire Healthcare NHS Foundation Trust, the same trust that treated Nottingham attacker Valdo Calocane, was involved in a fatal incident less than a year prior to Calocane’s violent rampage. The incident in August 2022 reportedly included an individual who had prior contact with the trust’s mental health services.

Paul Devlin, the former chairman of the trust, acknowledged a missed opportunity to prevent Calocane’s attack that resulted in the deaths of three individuals. Calocane, who had received care from the trust from 2020 until his discharge in 2022, carried out a brutal stabbing spree in June 2023, targeting students and a caretaker.

The inquiry highlighted that another former patient of the trust committed a fatal attack in August 2022, although details of the incident were not disclosed. The inquiry also criticized the lack of action taken by the trust’s board in response to the incident.

Despite being informed of the August 2022 incident in September of that year, the trust’s investigation was delayed until after a police inquiry was completed. An action plan related to the attack was only approved in May 2025. It was suggested that an audit of “unsafe discharges” in September 2022 could have potentially identified issues related to Calocane’s case and possibly prevented the subsequent attacks in June 2023.

Calocane, who pleaded guilty to manslaughter and attempted murder charges, is currently detained in a high-security hospital. The inquiry is ongoing as investigations continue into the series of incidents involving mental health patients under the trust’s care.
