Iran’s new supreme leader, Mojtaba Khamenei, has been absent from public view since the conflict with the US began, reportedly due to severe facial burns sustained in an airstrike. He has not made any public appearances or released any audio or video messages since he assumed power following the airstrike on February 28.

Tensions between the US and Iran escalated in late February after joint US-Israeli forces conducted strikes on key Iranian targets. In retaliation, Iran launched attacks in the region and disrupted the vital trade route through the Strait of Hormuz.

Despite sustaining critical injuries in the airstrike, Khamenei, aged 56, has remained mentally alert and engaged, as confirmed by Iranian officials familiar with his condition. He has undergone multiple surgeries to address his injuries, including procedures on his leg and hand, with plans for a prosthetic limb and eventual plastic surgery due to severe burns on his face and lips.

Given security concerns, senior government officials avoid direct visits to Khamenei to prevent potential tracking by Israel. Messages to him are delivered through handwritten notes sealed in envelopes and relayed through a trusted network of couriers to ensure his safety.

President Masoud Pezeshkian and the health minister are actively involved in Khamenei’s medical care. The supreme leader maintains limited direct contact with Iranian leadership, delegating decision-making authority to his generals. Political factions within Iran continue to engage in discussions, with experts highlighting Khamenei’s strong relationships with military leaders as a key factor in shaping current power dynamics.
