Noelia Castillo Ramos, under the care of Spain’s mental health system, endured a traumatic experience and a subsequent suicide attempt that left her paralyzed and in constant pain. She made the decision to end her life in Spain, a country that recently legalized euthanasia and assisted suicide for adults experiencing severe suffering.

Noelia’s father attempted to block the euthanasia through a right-wing Catholic campaign group, sparking a debate on assisted suicide. President Trump has now intervened, criticizing Spain’s systems. A leaked diplomatic cable reveals that the State Department instructed the US Embassy in Madrid to investigate how Spanish law enforcement handled cases of sexual assault, including Noelia’s.

The Trump administration has expressed grave concerns about the human rights failures that led Noelia to seek assisted suicide, highlighting the alleged disregard for her hesitancy. Despite Spain legalizing euthanasia for cases of non-terminal suffering years ago, questions remain about its application in cases involving psychiatric conditions.

Noelia, a victim of multiple sexual assaults, blamed Spain’s lax immigration laws for the attacks. In an interview, she mentioned that her family did not support her decision to choose euthanasia. Spanish government data shows a rise in assisted dying requests, with a case recently going to court for a judge to decide.
